Specialty Clinical Programs in South Plainfield, NJ

Some patients need more than standard skilled nursing. AristaCare at Cedar Oaks accepts medically complex patients that many facilities cannot, with a physician on site at least three days per week and a full-time nurse practitioner. The programs below reflect the clinical depth behind that capability.

American Heart Association Heart Failure Certified

Cardiac Program

AristaCare at Cedar Oaks is American Heart Association Heart Failure certified, a distinction few skilled nursing facilities in New Jersey have earned. The program is overseen by a dedicated Cardiac Practitioner alongside a cardiac medical director. Patients recovering from heart failure, cardiac surgery, or a cardiac event receive physician-led care with cardiac telemetry monitoring, including telemetry-monitored rehabilitation sessions. The program emphasizes safe recovery, medication management, and preventing hospital readmission.

Stroke Recovery Program

Stroke patients receive coordinated care from physical, occupational, and speech-language therapists working from a single plan, with regular physician review. Therapy addresses mobility, communication, swallowing, and the daily skills independence requires, and continues seven days a week.

Pulmonary and Respiratory Care

Our respiratory program supports patients with COPD and complex breathing needs, with respiratory therapy available around the clock. Capabilities include tracheostomy care and management, high-flow oxygen, BiPAP and CPAP support, nebulizer treatments, and percussion vest therapy, helping patients rebuild respiratory strength after pneumonia, COPD exacerbations, and extended hospitalizations.

Wound Care

Wound care at Cedar Oaks is physician-directed, with a wound physician and nurse practitioner conducting regular rounds. Services include wound vac placement and maintenance and bedside debridement, with treatment plans adjusted week to week based on healing progress.

Complex Medical Care

Cedar Oaks manages clinical needs that often keep patients in the hospital longer than necessary, including IV antibiotics and fluids, PICC line insertion and care, TPN, on-site blood transfusions, and on-site peritoneal dialysis. STAT lab work, X-ray, and EKG are available on site, so treatment decisions are not delayed by transport.
